Communicable Disease Protocols 

Forms 

On the day of your appointment, we will require you to fill out an online 'Client Screening Questionnaire' . This form ensures you are an appropriate candidate for in-person care, and are comfortable with the risks associated with an in-clinic appointment. You will not be able to enter the clinic until this form is completed. Please note that you will need to repeat this form before each consecutive appointment. If you do not see this form in your inbox the day of your appointment, please check your junk folder or contact us. 

Masks 

All clients who enter our space are required to wear a mask. You may bring your own mask with you, which can be either cloth or paper. A buff or bandana covering your face is also acceptable. If you do not have a mask, we can provide one for you. A donation to the food bank would be appreciated. When you enter the clinic, please ensure that your mask is on. If you require one from us, please indicate this to the receptionist so that they can have it ready for you.

Arrival at the Clinic 

Please arrive at the clinic at the time of your appointment. If you are early, we ask that you wait outside or in your vehicle. You will be asked to wear your mask and sanitize your hands upon entering. Our staff will direct you to the waiting room to meet your therapist. Remember to wear or bring appropriate clothing for your appointment, for example, shorts or a tank top. 

Personal Items 

Please leave all personal items (including jackets, water bottles, purses etc.) in your vehicle. If you will be arriving by bicycle, we ask that you lock your helmet to your bike, and leave any other personal items at home.

 

Others in Your Appointment 

No one will be allowed to accompany you to your appointment, unless you are under the age of 16 and require a parent/guardian, OR you would typically have an assistant with you for safety or mobility concerns. If someone will be accompanying you to your appointment, please let us know in advance so that we can ensure additional forms are completed by these individuals. 

Hand Sanitizing 

Once you enter the clinic, you must stop at the hand sanitizing station and sanitize your hands. 

Waiting Room 

Please take a seat or stand in the designated waiting room while respecting social distancing. We will make efforts to minimize your time waiting. Please

avoid touching any surfaces while you are waiting for your therapist to come get you. Your physiotherapist will also be wearing a mask. 

Appointment 

The appointment itself will take place in a private treatment room. Clients will not have access to our gym space at this time. This may mean that certain equipment may be brought into the room for your use during the appointment. All equipment is thoroughly sanitized after each use. 

Payment 

All billing and payments will occur without direct contact with the receptionist before your appointment begins. Payment methods will be secured in advance of your appointment, and then charged following each appointment. You will receive an electronic receipt via email. Your exercise program will also be emailed to you following your appointment.
